CONTRACT REQUIREMENTS:
Parents will need to provide the following prior to enrollment date…
Signed Contract (all custodial parents must sign)
Notarized Emergency Forms
Copy of Current Immunizations
Copy of Child’s Birth Certificate
Copy of Parents Driver’s License
Copy of Custody papers (if applicable)
One week refundable security deposit at time of enrollment
One week refundable security deposit within 30 days of enrollment
(Deposit is applied to your last two weeks of care with proper written intent to leave notice)
Two weeks tuition
$100 annual supply fee (due upon enrollment and then annually on or before June 1)
$7 Weekly Reader annual fee
ITEMS NEEDED FROM HOME:
Change of clothes in ziploc bag
Diapers/Wipes
Formula/Bottles if used
Baby food until able to eat regular lunches provided
Any needed medications
Pack -N- Play if under 2.5 years old
Sippy Cup w/stopper
Bathing suit, towel, and sunscreen in summer months
